A previously treated maxillary first premolar presenting with persistent apical pathology, a missed canal, and a pulpal floor perforation.

Mechanism of Action

The mechanism of action of EdgeUtopia™ BioCeramic Sealer is closely related to its interaction with tissue moisture and dentinal fluids. Once the calcium-based components of EdgeUtopia™ BioCeramic Sealer come into contact with moisture within the dentinal tubules, calcium hydroxide is formed.

This calcium hydroxide dissociates into calcium (Ca²⁺) and hydroxyl (OH–) ions. The hydroxyl ions increase local pH, contributing to the material’s antimicrobial effect. The released calcium ions then react with carbon dioxide in tissue fluids to form calcium carbonate (calcite).

This process supports the secretion of an extracellular matrix rich in fibronectin, stimulated by the alkaline environment and attracted to areas of calcite deposition. Histologically, this promotes hard tissue formation through calcite granulation, with fibronectin acting as a scaffold for cell adhesion, migration, and differentiation, ultimately supporting biomineralization and periapical healing.
